Making a Referral

A referral to First Link® will save you valuable time and energy.  It’s simple, just fax us the completed Referral Form and our First Link® team will take care of the rest. 

Your clients will receive:

• Telephone contact offering information and support.
• A package of information about Alzheimer’s disease and related dementias.
• The opportunity to register for our various programs and services.
• Linkages to appropriate community services.
