Social Work Support


We have two social work positions, urban and rural. The program has been developed to meet the needs of the London and Middlesex County community for people whose lives have been affected by Alzheimer's disease and related dementias.

 

What Does The Social Work Program Offer?

Our Social Workers will provide in home assessment to determine individual and family needs and work with all those involved to develop an individualized plan of care for the person living with Alzheimer's Disease or a related dementia. The program also offers individual and family counseling, family mediation, case conferencing, and support groups. The social workers can provide you with information on community resources as well as referral and advocacy.

The services provided by the social workers are free for individuals with Alzheimer's disease and related dementias and for their families and caregivers living in London and Middlesex County. The information shared is kept confidential within the limits of the law.
